CLO Case Management brings over two decades of Case Management Services!

CLO’s Targeted Case Management (TCM) services are aimed at helping you achieve the quality of life you deserve! It is staffed with a group of professional, extremely well trained case managers ready to assist you as you navigate the ever-changing waters of services and transition. With Medicaid tightening the definition of case management services, you will find CLO’s TCM Department expanding the definition of quality services for those they support. If you are looking for a case manager for yourself, a friend, a loved one or ward, consider CLO’s TCM Department.

CLO’s case managers will walk with you every step of the way in finding the services that best suit your needs and interests.  Your case manager will develop an understanding of your needs and interests and aid you in finding the appropriate supports that will best fit your desired lifestyle.  

Your CLO case manager will work closely with you and your Managed Care Organization care coordinator and the Kansas Department Division of Children and Families (DCF) and Kansas Department of Aging and Disabilities (KDADS) to ensure you retain your benefits (i.e., services, Medicaid and food stamps).  If you have a Housing and Urban Development voucher, your CLO case manager will assist with the annual redetermination process and/or assist with getting you on the waiting list for affordable housing. They will assist you with obtaining benefits and locate additional assistance or resources as needed. 

Transitions and change can be frightening, but change is important for growth. Our case manager is available to walk you through transitions and work with you, your family, your school services, your service provider, and your Managed Care Organization to offer you support.  Many options are available throughout your lifetime, including school, a job, a new home, or new services.  Your case manager will work closely with you and your team to make these changes a little bit easier.

We at CLO view each person as an individual with different interests and support needs.  The services we provide are very individualized.  We get to know you and your needs so that we can help you live the quality of life that you deserve.
