CLO’s Targeted Case Managers (TCMs) help ensure that each individual we serve obtains and benefits from all the services and supports offered within CLO, the community and the government. CLO’s TCMs serve as facilitators for of each individual’s support team. CLO employs professional, well-qualified and dedicated case managers who are committed to achieving high quality outcomes for the individuals they support by helping families through:

• Assessing the person’s needs and lifestyle preferences
  
• Providing transition planning to promote success in new environments
  
• Facilitating the person’s person-centered support planning process (called a Quality of Life Plan at CLO)
  
• Pursuing resources, funding, services and supports to meet the needs, interests, goals and preferences of the individual
  
• Evaluating the quality of services provided and taking action to achieve desired outcomes
  
• Serving as a liaison and communicator with families and guardians
